Affiliate	□ Affiliate $100 (Businesses not in the groundwater industry, but participate in CGA offered programs {e.g. insurance})
Government	□ Individual $85 (Covers only single employee in agency) □ Agency $375 (Covers multiple employees in agency)
Business	□ Business $375 (Groundwater businesses)
